While casual or moderate drinking has some potential advantages – relaxation, heightened enjoyment of stimuli, etc. – problem drinkers are unable to enjoy these advantages without alcohol. In other words, casual or moderate drinkers will be able to find other ways to relax or enjoy themselves even if alcohol is not present, or they make the choice to abstain. But if someone is completely unable to function for pleasurable reasons without alcohol, they cannot conceive of having a good time without getting drunk, or reaching for the bottle is their first response to any kind of trigger (either stressful or pleasurable), then this is a sign that they are abusing alcohol and need help to stop. A large body of scientific evidence has been gathered in recent years to show that addiction can run in families. In fact, children of alcohol-addicted parents are four times more likely to develop alcohol addiction in later life than those born to parents without alcohol addictions. Where alcoholics are concerned, their brains have become so accustomed to dealing with alcohol that the volume of chemicals being produced to overcome the effects of alcohol is excessive. As blood alcohol levels start to fall, those same brain chemicals start causing unpleasant withdrawal symptoms. The only two solutions are to either consume more alcohol or wait it out until the body readjusts.
Crucially, DBT is also collaborative: it relies upon the ability of the addict and therapist to work things out together interactively. DBT is broken down into four modules – Mindfulness, Distress Tolerance, Emotion Regulation, and Interpersonal Effectiveness – which is an approach which allows addicts to focus on one particular task or aspect of themselves at once, and enables the therapy to be targeted more acutely at the individual addict and their own particular situation.
During the early stages of alcohol recovery, patients can be confused and scared. Their emotions can run high to the point that what they are thinking and feeling interferes with recovery. Meditation addresses these sorts of things. By helping patients relax and focus their thoughts inward, meditation eases patient fears and clears up confusion. The National Institute of Mental Health defines depression as a common but serious mood disorder that causes severe symptoms that can affect how you feel, think and handle daily activities. Sometimes depression precedes substance abuse, and other times, the reverse is true. Recovery from alcohol addiction is a lifelong journey. You may face relapses and temptations for most of your life. It’s not uncommon to slip in and out of sobriety as you work your way through your addiction. Some people beat addiction the first time they try to become sober. Others battle alcohol dependence for many years. The more you try, the higher your chances of success.
An influential cognitive-behavioral approach to addiction recovery and therapy has been Alan Marlatt's (1985) Relapse Prevention approach. Marlatt describes four psycho-social processes relevant to the addiction and relapse processes: self-efficacy, outcome expectancy, attributions of causality, and decision-making processes. Self-efficacy refers to one's ability to deal competently and effectively with high-risk, relapse-provoking situations. Outcome expectancy refer to an individual's expectations about the psychoactive effects of an addictive substance. Attributions of causality refer to an individual's pattern of beliefs that relapse to drug use is a result of internal, or rather external, transient causes (e.g., allowing oneself to make exceptions when faced with what are judged to be unusual circumstances). Finally, decision-making processes are implicated in the relapse process as well. Substance use is the result of multiple decisions whose collective effects result in a consumption of the intoxicant. Furthermore, Marlatt stresses some decisions—referred to as apparently irrelevant decisions—may seem inconsequential to relapse, but may actually have downstream implications that place the user in a high-risk situation.
Urge surf. Many people try to cope with their urges by toughing it out. But some cravings are too strong to ignore. When this happens, it can be useful to stay with the urge until it passes. This technique is called urge surfing. Imagine yourself as a surfer who will ride the wave of your drug craving, staying on top of it until it crests, breaks, and turns into less powerful, foamy surf. When you ride out the craving, without trying to battle, judge, or ignore it, you’ll see that it passes more quickly than you’d think.
At the end of an intervention, the stage is set for entry into addiction treatment programs. There are many different options out there. Some facilities, for example, offer inpatient treatment for addiction. These programs allow people to step away from their day-to-day concerns and tackle an addiction around the clock, every single day. For some people, that tight focus is an ideal setup for healing. But outpatient centers can be ideal for those who want to stay at home, surrounded by family, while they work on addictions to alcohol. It’s a personal decision that families can make in consultation with the person who needs help.
