Instead, you should follow the procedures and mechanisms worked out during your therapy, and take all steps agreed upon to minimise your exposure to risk. During therapy you will have worked to identify triggers which can set off the desire to consume drugs; now, in the outside world, it is your responsibility to avoid those triggers in any way possible.
Before entering a rehab facility, patients may have to undergo detox treatment. Detox is the process in which a patient rids his or her body of the addictive substance. From start to finish, this process varies in length, but often takes about a week. As part of a medical detox program, recovering patients will be monitored by doctors and nurses and given medications to manage withdrawal, when appropriate. Once a patient completes detox, he or she is ready for rehab.
Changes in the brain that support physical and psychological dependency on mind-altering substances are the direct cause of addiction, but those changes do not occur at random. Addiction experts believe drug addiction emerges from an interplay of genetic and environmental factors, although one factor or the other may be strong enough to make a person vulnerable to addiction in some instances. One study tracked the weekly drug use among individuals who attended residential treatment centers. After one year post discharge they discovered that there is a correlation between retention rates and the length of stay at a facility. Individuals coming form programs of 90 days or more showed a lower relapse rate than those coming from programs of less than 90 days.
Individual therapy will help you learn to recognize triggers and cope with them. The therapists may also help you to improve your emotional regulation skills in order to better avoid relapse. Group counseling provides you with the opportunity to practice sober social skills, as well as the coping strategies you learned in individual counseling. Family therapy sessions can help to repair broken relationships, improve communication skills, and build conflict resolution skills. Medication, such as methadone or Suboxone, may be used in combination with behavioral therapy to help opioid-addicted individuals remain abstinent. Once your rehab program nears an end, your treatment team will create an aftercare or relapse prevention plan for you consisting of ongoing support. Ongoing support may include individual therapy, group counseling, self-help group meetings (e.g., 12-step, SMART Recovery), alumni programs, or sober living homes.1,2

Most countries have legislation which brings various drugs and drug-like substances under the control of licensing systems. Typically this legislation covers any or all of the opiates, amphetamines, cannabinoids, cocaine, barbiturates, benzodiazepines, anesthetics, hallucinogenics, derivatives and a variety of more modern synthetic drugs. Unlicensed production, supply or possession is a criminal offence.

Nalmefene, an opiate antagonist that is similar in its chemical structure to naltrexone, is one of the most recent drugs being investigated for the treatment of alcoholism. Like naltrexone (sold as ReVia, Depade, or Vivitrol), nalmefene deprives the person struggling with substance use of the pleasurable feelings associated with drinking. But nalmefene is less toxic to the liver than naltrexone. As of 2013, nalmefene was still undergoing clinical trials through the U.S. National Institutes of Health before receiving FDA approval.
